Authorization Forms
Be sure to fill out the appropriate forms before you visit the salon for your appointment! You'll need to fill out the New Client Consultation Form and Hair Salon Waiver Form. If you're coming if for lash extensions, we also ask you to fill out the Eyelash Extension Consent Form.
